Moving into or between different care settings or services – such as home care, care homes, or extra care housing – can be difficult for older adults as well as for the family members and friends who assist them.
By identifying key challenges faced by those involved in care moves, and learning lessons from good practice, we can propose strategies to improve practice.
This has been the goal of the Better Care Moves for Older People project – funded by the National Institute for Health and Care Research School for Social Care Research (NIHR SSCR) with support from the NIHR Applied Research Collaborations (ARCs) Kent, Surrey and Sussex, and North Thames.
The project team – Dr Wenjing Zhang, Research Fellow at the University of Kent’s Centre for Health Services Studies, and Dr Eirini-Christina Saloniki, Senior Research Fellow in Health Economics at University College London (UCL) – have been engaging older people, family carers, and social care practitioners as co-researchers in this area.
The goal was to provide practitioners and stakeholders with evidence-based resources to guide and support these care transitions.
Alongside their online video-based resources, the team has developed the following resources:
- a short policy brief for social care staff and managers that provides practical tips for improving move-related practices
By sharing and learning from these resources, we can improve the experiences of older people, their loved ones, and their support teams.
